Why was the CMAAP created?
The CMAAP program was created to enable community pharmacists to provide full medication adherence services to their patients. Consistent with WPQC Level 2 guidelines, the CMAAP program is a step-by-step process to identify key barriers that prevent medication adherence by patients, and related “Talking Points” that prompt pharmacist’s coaching to overcome those barriers.
What is the benefit of this pilot?
The pilot will enable participating pharmacists to provide consistent Medication Therapy Management services to patients, maintain a history of each patient-pharmacist interaction and generate a Medication Action Plan (MAP) for the patient. Finally, during the pilot, payers will reimburse participating pharmacies for each consultation completing using the CMAAP.
How long will the pilot last?
The pilot is scheduled to last for six months.
Why use the CMAAP System?
Using the system makes it easy for pharmacists to complete a comprehensive patient consultation quickly, address all issues and concerns the patient has. In nearly all cases, data entry requires just one click of the computer mouse, significantly reducing the time needed to create a full patient record of each interaction. Users can maintain a history of each patient-pharmacist interaction and generate a Medication Action Plan (MAP) for the patient. CMAAP is a highly intuitive, easy to use system that requires little training. The application allows pharmacists to use their own clinical expertise to drive the consultation, while the application simply helps structure the consultation and aggregates the data that is being collected during the patient-pharmacist interaction.
When do I use CMAAP?
CMAAP should be used whenever you have an opportunity to provide a comprehensive Medication Therapy Management coaching session to improve adherence with medication taking.
How do I identify a patient for participation?
Every patient who qualifies for a Level 2 consultation under WPQC guidelines is eligible for care using the CMAAP system. Key opportunities for use include diabetes patients, those taking four or more medications for chronic conditions, anyone whose record shows an erratic purchasing history for long term medication, and new Medicare customers.
How do I invite the patient to participate?
There are a variety of ways to extend an invitation for patients to participate in the CMAAP program. We think that most pharmacists will follow their own best judgment about how and when to invite a patient to make an appointment. There are several effective approaches you can use:
- When the patient is at the pharmacy counter. When a patient is in the store to fill or pick-up a prescription, you can use the opportunity to introduce the program and invite the patient to participate in the program. You can attach a note to remind them of the date and time for the appointment to the medication bag and a flyer that explains how they can benefit by participating. Flyers will be provided to each store for this purpose.
- Mailing an invitation from the pharmacy is another opportunity to make contact with patients and extend an invitation for them to participate. Invitations will be provided to each pharmacy and you can use the printed invitation for mailing to the patient, and you can include the descriptive flyer that describes the program in more detail.
- A personal telephone call to the patient’s home is another effective way to introduce the CMAAP program to appropriate patients. During the call, you can briefly explain the program, invite patients to the store for a consultation, and set a date and time for your meeting with him or her. It’s a good idea to send a reminder by mail, and include the flyer that explains the program in greater detail.
How often do I follow up with each patient?
The CMAAP system allows you to use your best clinical judgment as to how soon after a consultation the patient should return for follow-up. The automated scheduling feature also allows you to schedule the date and time for the next consultation. The information is then printed on the MAP which the patient takes home.
How much time does each consult session take?
Patient consultations in the CMRA mode should take between 20 and 30 minutes to complete. Consultations are scheduled in advance with the patient, so pharmacists know in advance when they will be meeting with a patient and can plan their work flow to be available for each patient. Consultations using the shorter adherence-specific Module take less time.
